Description

This workshop is the third in a series that focuses on the practice implementation of the benchmarks for the SWAN Family Matching Unit of Service. The first two sessions focused on completing Family Matching unit forms, collecting and exchanging information and the selection process. This third session will look closely at the Family Matching Extension, Family Matching Assessment for Unmatched Families and the Child/Family Match Confirmation form.
