Description
This workshop is second in a series that focuses on practice implementation of the SWAN Family Matching Benchmarks. The initial session focused on completing the Family Matching Plan and the process of exchanging, requesting and reviewing information. This second session will take an in-depth look at the selection process after a child is identified. This includes providing family support during decision-making, the county interview process and Post Adoption Contact Agreement implications. We will also focus on the completion of the Predictions Form.
