Description

When a child or youth is placed into a pre-adoptive home, it is a time of adjustment for both the child and the family, which can be challenging for all involved. This workshop provides an overview of the SWAN Finalization Unit of Service, including benchmarks and use of the Individual Permanency Service Plan, and explores how it can strengthen and support families and children as they navigate toward permanency.
